Where and when does a removed or bolstered unit's counter come back?

On its spawn marker, not where it fell, and only when a card for that unit is played. Bolstering only puts cards into your discard pile; the counter is placed when you take a card action with it, and that unit then acts straight away. Spawn points work even if the enemy controls or stands on them.
