What happens when a hero is defeated? Is he wounded first?

Yes. A healthy hero who is defeated is wounded: he discards all damage and flips to his wounded side. If a wounded hero is defeated he withdraws and is removed from the mission. Excess damage from the hit that defeats him is ignored, the wounded side starts at zero damage, and he stays wounded until the mission ends.
