Will I always start Act 1 with exactly 3 cards in hand?

No. Three is the usual number, but a card played in setup that offers a double Operations increase (you discard another card for the second step), or the Docks bonus of a 2-player game, can leave you with fewer or more than 3. Nothing forbids taking those bonuses during setup.
