Can I take a second Operations bump during the Setup Round, and what Infamy does that give?

Yes. If you open a Speakeasy that gives an increase and then play an Operations card that allows a second increase (discarding a card for it), that is legal. Infamy is your starting 5 plus one per level gained, so three increases put you at 8, and reaching 8 gives the Family Member on the Infamy track.
