Do Casinos still pay out if the Restaurant visitor keeps the same turn order position?

Yes. Turn order counts as changed whenever a marker is moved to the lower row, even to the same position, so any Restaurant visit triggers the payout. This also means a last-place player in a 2-player game can go to the Restaurant, pay the card cost and stay in place just to trigger Casino income.
