Do I pay to use a Mobster's bonus, or only to add their strength?

You pay only when you hire your Associate to add strength to a Mob War defense or a Speakeasy takeover. Ongoing bonuses (such as Owney Madden's, or the +2 permanent Leverage and +1 Rum-runner Attack strength Associates) and instant bonuses are free. One-use Mobsters stay City side up until used and then flip.
