When an attack breaks several locations of a large enemy, does the stage card reaction happen for each one?

It depends on how the damage is dealt. If the damage is applied simultaneously, such as Chain Lightning, Ice Spike, or Fireworm, several dice can break at once, and if the enemy dies, every break except the last one triggers a stage card. If the damage is sequential, as with an ability that attacks twice, only one die can break because the remaining damage is stopped.
