How do the Knee and Hand Serious Wounds change Action costs?

The Knee raises the Cost of your first Movement Action (Escape included), but only if you take it in your first turn of the round; if you do something else first and move later, you pay the normal Cost (the FAQ confirms this). Players also apply it to other Movement-type Actions such as Careful Movement or Vents, and Actions that already cost 2 stay at 2. The Hand wound adds to Item actions but not to Shoot, which is a basic action; Items that print their own Cost are affected.
