Does an effect that lowers Sanity spent reduce Sanity lost to Mythos cards or combat?
No. Spending Sanity is an optional cost and losing it is a different thing, so a "spend less" discount does not reduce loss from a Mythos card or combat. Tattered Cloak instead reduces Sanity lost due to an effect, which players accept covers Mythos card text; whether combat damage counts as an effect is argued.
