Birth of Madness Campaign Introduction
A low fog falls the streets of Yharnam this night, and the air itself hangs thick. As you traverse the alleyways and darkened corners of the city, you are met by the sight of numerous townsfolk wandering the streets, incoherently and oblivious to your presence. Listening, you notice an unsettling repetition through their deranged ramblings: one phrase, repeated: "Woe to the False Moon... The infant has been born... The infant has been born..."

Birth of Madness Mission Cards
Faces are listed in the order they appear on the scan. The title and flavour opening are given to tell apart cards that share a title.
Hunt Mission – Birth of Madness ("As you traverse the streets")
As you traverse the streets, you cannot shake a growing uneasiness that sits at the edge of your Hunter's instincts... Though the afflicted are seemingly more docile than the usual frenzied hordes, the reach of the madness is great this night... No matter. There is work to be done and the night's Hunt is only beginning.
Collect at least 2 Insight. Reveal card 2.
Hunt Mission – Birth of Madness ("You must discover the purpose")
You must discover the purpose of the strange creatures. A sharp pain rings through your head, and visions of a sprawling tower fill your mind. In the distance, you hear what sounds like the faint cries of an infant...
Place [⊕] +1 Insight tokens on this card. Replace all Hunter Mob Enemies on the map, and their card, with the Mergo's Attendant Enemy. After each [red orb], spawn 1 Mergo's Attendant (if available) on each [blue orb] space, beginning with those closest to Hunters. Each time a Mergo's Attendant is slain, remove 1 token from this card.
When all tokens have been removed from this card: Reveal card 3.
Hunt Mission – Birth of Madness ("The ever-present low crying")
The ever-present low crying grows in pitch. You grip your head in pain. Your vision blurs as you see images of a place deep in the heart of the district and a large figure emerging from the fog. Somehow, you know exactly where to go to face this creature...
Reveal card 4.
If the Courtyard Lamp tile is not in play, search the tile deck and place it. Surround the Courtyard Lamp tile with Fog Gates. Spawn 1 Mergo's Chief Attendant on the Courtyard Lamp space. It respawns to this space on [red orb].
Complete the Hunt by slaying this Chief Attendant: Reveal card 5.
Herald
Place 1 Insight token on this card. If 3+ Hunters, place 1 additional token. When the Chief Attendant would be slain, instead, remove 1 token and Heal all [red drop] from it. All tokens are returned on [red orb].
Each time Mergo's Chief Attendant Activates, spawn 1 Mergo's Attendant on its space. These Attendants do not respawn on [red orb].
Entering the Nightmare
Where your blows land, a strange wave seems to ring out from the creature, as if causing ripples in reality itself. As the creature falls, pain grips your head, the world spinning around you. Darkness overtakes you as you collapse to the ground.
Distribute 1 Blood Echo and the Messenger's Gift reward among the Hunters.
Insight Mission – Those in Need ("From the window, a weak female voice speaks")
From the window, a weak female voice speaks. "Hello...? Is someone there...? Please... It hurts so much... I can barely move... Sedatives... Anything... Please... I cannot stand it..." Whomever she may be, this woman sounds to be in tremendous pain.
While in the Dream, you may discard 1 Consumable to search the Consumables deck or discard pile for 1 Sedative card. Shuffle the deck afterwards.
Interact on the Barred Window space and discard a Sedative Consumable: Reveal card 7.
Insight Mission – Those in Need ("Knocking on the barred window")
Knocking on the barred window, you see the silhouette of the woman. "Y... You've actually returned...? Oh thank you, kind Hunter! The pain is so great... And the crying... It won't stop... But... This will make it all better, won't it? It has to... It has to..." With this, she disappears from view. Suddenly, your senses flare. Swiftly turning, you come face to face with a strange creature that appears from nowhere.
Spawn 1 Mergo's Chief Attendant on the Barred Window space. It respawns to this space on [red orb].
Complete this Mission by slaying this Mergo's Chief Attendant: Reveal card 8.
Otherworldly Influences
The creature's form erupts into a thick fog, which swiftly rises to cover the area in a choking haze. As it is about to envelop you, the street is suddenly bathed in the red rays of the Blood Moon. Where it touches, the fog seems to recoil, burning away...
Distribute 1 Consumable and the Caryll Rune: Moon reward among the Hunters.
Insight Mission – Profane Harvest
Numerous bodies of townsfolk lie strewn about. In the darkened interior, you see motion. Two beings, reminiscent of metallic statues, stand over the corpse of a woman. More horrific, a worm-like creature lies partially emerged from her corpse.
Reveal card 10.
Surround the Ransacked House tile with Fog Gates. Spawn 2 Mergo's Attendants and place 1 Insight token (Celestial Child) on that space. They repawn to this space on [red orb]. [sic: "repawn" as printed]
Complete this Mission when no Enemies remain on this tile: Reveal card 11.
Celestial Child (enemy card)
The Celestial Child has 1 HP.
Any time an Enemy within 1 space of the Celestial Child (including itself) flips Ability or Special, they also Block 2 [red drop].
The Child only Attacks when Basic is flipped:
Basic: Horrific Cries – icons: three arrows [>>>] and one [red drop]. Frenzy. Cannot be Dodged. Targets all Hunters within 2 spaces.
Profane Visions
You move to inspect the corpse of the creature. Touching it, your mind is suddenly bombarded with visions of some far off place: A sprawling tower of impossible geometry. From it, you feel the gaze of something beyond your understanding reaching into your mind.
Distribute the Caryll Rune: Communion reward and the Hunter Blunderbuss firearm among the Hunters.
Insight Mission – Unknown Motives
Entering the clinic, you see a curious sight. A stone-like figure is rummaging through the area. It does not appear hostile. Retrieving a container, it turns and slowly lumbers past you. Truly baffled by this behavior, you decide to follow this creature.
Spawn 1 Mergo's Attendant on the Iosefka's Clinic [blue orb] space. It does not Attack or Activate as normal. At the end of each round, if no Hunter is within 2 spaces, remove it from the board and discard this Mission. Otherwise, it moves 2 spaces toward the Central Lamp tile.
Complete this Mission when the Attendant moves onto the Central Lamp tile: Reveal card 13.
Exiting the Waking World
The creature's methodical steps come to an abrupt halt as it enters the crossway. Its form begins to dissipate into the surrounding fog. Darting forth, you strike only empty air as it fades from sight... Just as that of a Hunter returning to the Dream...
Distribute the Caryll Rune: Eye reward among the Hunters.
Mergo's Loft (tile card, first)
Take and place the Mergo's Loft tile within reach of all players.
Hunters do not begin the game on the Central Lamp tile (it is still placed during setup). Instead, they begin on the Mergo's Loft [blue orb] space. They may only respawn to this [blue orb] space.
Lower Level
(Tile card; the tile is shown on the card.)
Populate this tile as shown above. Instead of the usual Enemy, spawn Mergo's Attendants on the [blue orb] spaces.
Hunt Mission – The Tower
You awaken in a daze, finding yourself in what appears to be some sort of torture chamber. Surrounding you lie many scattered and broken cages which appear to function as lifts. Before you have time to properly explore the area, dark figures advance upon you.
When no Enemies remain on the Mergo's Loft tile: Reveal card 17.
Hunt Mission – Ascending the Tower ("The last of the creatures falls")
The last of the creatures falls, allowing you time to now explore the area. From a nearby window, you catch a glimpse of the Blood Moon, risen high into the night. While this is usually an ill omen, for some reason, as its rays bask over you, you feel an eerie calm... and a compulsion to climb the tower. You find a barred and locked lift, seemingly the only method of ascending. Inspecting it, visions of places across Yharnam and the large creatures from before fill your mind. As they pass, you find yourself back within Yharnam, some force compelling you onward...
Discard card 14. Reveal card 18.
End a move on the Oedon Chapel tile: Reveal card 19.
Access to the Tower
Move all Hunters to the Central Lamp tile.
Hunters may not access the Mergo's Loft tile until they have collected at least 2 Insight this Chapter. Once they have, they may access the [blue orb] space from the Hunter's Dream or Central Lamp space.
Hunt Mission – Ascending the Tower ("Something compelled you here")
Something compelled you here. Though what, you do not know. Entering the old chapel, you find nothing out of the ordinary. Yet, as you inspect the hallowed place, your vision begins to blur. You realize quickly, however, your vision is not what is being affected, rather the building itself appears to blur. Slowly, the figure of one of the large creatures begins to manifest. Once fully formed, it turns as if confused to its surroundings. Noticing you, it raises its large cleaver to attack.
Spawn 1 Mergo's Chief Attendant on the Oedon Chapel space. It is not removed on [red orb].
Slay this Attendant: Reveal card 20.
Hunt Mission – Ascending the Tower ("As the creature falls, you notice them")
As the creature falls, you notice them; the cries from before. You had almost forgotten them, as if they had merged into the background of your mind. But now they return, piercing your ears and thoughts, pounding away at your very sanity. You stagger, swiftly moving to the chapel exit, something urging you to get out. As you emerge, you nearly collapse in the street. Yet, as soon as you are out of the building, and once more in the bathing glow of the Blood Moon, peace returns. Looking down, you find an old key clutched in your hand... Let us continue...
End a move on the Mergo's Loft tile: Reveal card 21.
Hunt Mission – Ascending the Tower ("Through the haze")
Through the haze, you arrive into the lower reaches of the tower, but you find you are not alone. A figure stands before you, flanked by the smaller creatures you have encountered through Yharnam. "Hmm...?" The figure turns. "So, the Presence sends its pawns..." They draw their weapon, as do the smaller minions flanking them. "For the sake of the child... I'll send you back to your precious Dream..."
Spawn 1 random NPC Enemy on the center space of the Mergo's Loft tile. It respawns to this space on [red orb].
Complete the Hunt when no Enemies remain on this tile: Reveal card 22.
Accessing the Upper Levels
Distribute the Caryll Rune: Hunter reward and the Repeating Pistol firearm among the Hunters.
Your foe staggers to a knee. "Heh... We all have our masters to serve... I hope you have chosen yours wisely..." They speak before falling back into the gaping chasm... Nonsense. All that is left is to move forward with your goal: Ascend the tower. Slay the infant.
Insight Mission – Fanatics
Pyres fill the ancient graveyard. Surrounding the massive tomb a congregation of townsfolk have gathered. "Praise to Formless Oedon. Praise to the Child!" An unknown fury swells within you, compelling you to attack.
Reveal card 24.
Surround the Tomb of Oedon tile with Fog Gates. Spawn 1 Hunter Mob on its [blue orb] space. It respawns to this space on [red orb].
Complete this Mission by slaying this Hunter Mob: Reveal card 25.
Fanatical Zeal
Place 2 Insight tokens on this card. If 3+ Hunters, place 1 additional token. When the Hunter Mob would be slain, instead, remove 1 token and Heal all [red drop] from it. All tokens are returned on [red orb].
Replace its Special with:
Special: Zealous Fervor – icons: arrow [>] and one [red drop]. Frenzy. This Attack gains +1 [>] and +1 [red drop] for every [red drop] on this Enemy.
Slaying the Followers
The last of the mob falls, leaving only you, the distant newborn's cries, and the bask of the Blood Moon. Infected blood sprawls across the tomb, tainting the very ground... But you have done well and shown your might to those who would oppose the Dream...
Distribute the Caryll Rune: Blood Rapture reward among the Hunters.
Insight Mission – Servants of the Enemy
A deep, almost instinctual hatred stirs within you for the ones serving this new threat. Hunting them down, like all beasts, is second nature to you. However, the compulsion to demonstrate your devotion through blood and fire is strange... Yet you are compelled.
During their turn, Hunters may discard 1 Blood Echo to spawn 1 Mergo's Chief Attendant in their space. Each time a Mergo's Chief Attendant is slain, gain 1 Corpse token. Hunters may discard their held Corpse tokens while on [blue orb] spaces, placing them on this card.
Complete this Mission when this card has [⊕] +1 tokens on it: Reveal card 27.
Tribute to the Blood Moon
The pyres burn high into the night sky. Far above, the crimson Blood Moon glows and, from deep within, you feel a satisfaction in slaying these servants. The cries that fill the night seem to intensify, but you are growing to enjoy their wail.
Distribute 2 Consumables and the A Call Beyond reward among the Hunters.
Insight Mission – Barred Window
If you have Otherworldly Influences: Reveal card 29.
Otherwise: Reveal card 6.
Insight Mission – In Need of Aid ("H...Hunter? I'm afraid my condition has only worsened")
"H...Hunter? I'm afraid my condition has only worsened... Heh, what a night for sickness... Could you-- Would you... Possibly consider helping me...? I need to get to the clinic... The pain... I simply cannot bear it any longer..." The young woman exits, gripping her stomach.
Place 1 Survivor token on the Barred Window space. Any Hunter may pick up the token when they move out of that space. The token is returned to the Barred Window space if the Hunter teleports or goes to the Dream.
End a move on the Iosefka's Clinic tile with the token: Reveal card 30.
Insight Mission – In Need of Aid ("As you approach the clinic")
As you approach the clinic, the pale fog swells around you. The young woman grips close to you, barely able to stand on her own now. From the fog, figures begin to emerge...
Reveal card 31.
Spawn 1 Mergo's Attendant on the Iosefka's Clinic [blue orb] space and mark it with an Insight token. This Attendant represents the Attendant Horde. It respawns to this space on [red orb].
Complete this Mission by slaying this Mergo's Attendant: Reveal card 32.
Attendant Horde
Place 1 Insight token on this card. If 3+ Hunters, place 1 additional token. When the Attendant would be slain, instead, remove 1 token and Heal all [red drop] from it.
During the Enemy Activation, it makes a number of bonus Activations equal to the remaining tokens on this card. During its Activation, it always moves toward and Attacks the Hunter with the Survivor token.
The Chosen (reward card, first)
The last of the horde falls, and with it, the thick fog covering the clinic dissipates. You guide the young woman to the clinic doors. "Thank you, kind Hunter." She smiles through obvious pain before entering the old building. Hopefully, she will find aid within...
Distribute 1 Blood Echo and the Iosefka's Blood Vial reward among the Hunters.
Mergo's Loft (tile card, second)
Take and place the Mergo's Loft tile within reach of all players.
Hunters do not begin the game on the Central Lamp tile (it is still placed during setup). Instead, they begin on the Mergo's Loft [blue orb] space.
Upper Level
(Tile card; the tile is shown on the card.)
Do not populate this tile at the beginning of the Hunt. On the first [red orb], populate this tile as shown above.
Hunt Mission – Ascending the Tower ("As you near the final lift")
As you near the final lift, you feel a presence invading deep into your mind, cautioning you to be ready before continuing. Whatever it is, you both harbor the same goal: find the source of this nightmare and strike it down!NBut... why are you so sure of this...? What knowledge led you here...? You shake such thoughts aside. We must hunt!
If all Hunters are on the Lift space at the end of a Hunter Turn, they may choose to ascend the tower. There is no returning. You feel that gathering additional Insight before ascending may aid you immensely.
Ascend: Reveal cards 36 and 37.
The Tower (tile card)
Replace the Mergo's Loft tile with the Wet Nurse's Luminarium tile.
Hunters can only respawn on the Wet Nurse's Luminarium [blue orb] space.
Unless Hunters have gained at least 2 Insight this Chapter, their hand size is reduced to 2.
Unless Hunters have gained at least 1 Insight this Chapter, their Attacks and Dodges suffer -1 [>].
Hunt Mission – Ascending the Tower ("The lift draws you higher")
The lift draws you higher and higher until you find yourself exiting into a massive area at its peak. In the center lies a solitary baby carriage, so out of place among these ruins. Here lies the source of the infernal cries. As you approach, a massive figure lands before you. Its form you cannot identify, but the rags draping it and the many blade-wielding arms tell you all you need to know: One more foe to slay.
Spawn Mergo's Wet Nurse in the center space of the Wet Nurse's Luminarium tile. It respawns to this space on [red orb].
Complete the Hunt by slaying Mergo's Wet Nurse: Reveal card 38.
Nightmare Slain
With its final guardian slain, you approach the carriage. While you see no visible form within, you can sense the presence you have been seeking. With a single blow, you shatter the entire frame to the ground. As you do so, the last escaping cries echo directly into your mind. As they fall silent, you turn to see the Blood Moon, now radiant and full in the night sky. Was it always there...? As its rays bask over you, the pull of the Dream beckons you return after a task well done.
(No printed rules text.)
Insight Mission – Gaining Favor ("Another who would come to oppose us")
"Another who would come to oppose us? Can't you understand? This is a new beginning. He can free us all! I can't let you stop that."
Spawn 1 random NPC Enemy on the Grand Cathedral [blue orb] space. They are not removed on [red orb]. They gain 1 bonus Activation at the end of each round, moving 2 spaces toward the nearest Hunter with the lowest HP and Attacking.
When this NPC Enemy is slain:
If you have Tribute to the Blood Moon: Reveal card 43.
Otherwise: Reveal cards 40 and 41.

They gain a bonus Activation at the end of each round, moving 2 spaces toward the nearest Hunter with the lowest remaining HP and Attacking.
Insight Mission – Gaining Favor ("Your blows strike true")
Your blows strike true, cutting your foe down. Yet, after an instant, they rise once more, restored. "You have your precious Dream. I have my master. Let us see which one is more favored!" There is some influence tethering them here... And what did they say about our connection to the Dream? A feeling in your mind tells you that is a key. We are strongest near the Lamps, our connection... Perhaps that is where they may be weakest?
Complete this Mission by slaying this NPC Enemy while they are on a [blue orb] space: Reveal card 42.
Proven Dedication
Once more, you strike down your adversary, and just like before, their form rises. Suddenly, the usual dull glow of the Lamp sparks bright and they stagger, as if their breath was suddenly pulled from them. Lifeless, their form falls to the ground. They will rise no longer.
Distribute the Old Hunter's Bone and the Tiny Tonitrus rewards among the Hunters.
Further Tributes
Your blows strike true, cutting your foe down. Yet, they begin to rise once again. For a moment, you even see their wounds beginning to close. Suddenly, however, they stagger, as if some force has gripped them, drawing the very life from them. Motionless, they fall to the ground...
Distribute the Old Hunter's Bone and the Tiny Tonitrus rewards among the Hunters.
Insight Mission – Defiling the Chapel
"The Great One Oedon, lacking form, exists only in voice." This monument to him deserves to be destroyed... Wait, but why...? No. Not why... How. Let us burn it to ash and cinder...
Place [⊕] Insight tokens on this card. Hunters may discard 1 Molotov Cocktail or Fire Paper while on the Oedon Chapel tile to remove 1 token from this card. While in the Dream, Hunters may discard their held Consumables to search the Consumables deck or discard pile for Molotov Cocktails or Fire Paper. Shuffle the deck afterwards.
Complete this Mission when this card has no remaining tokens: Reveal card 45.

Insight Mission – Iosefka's Clinic
If you have The Chosen: Reveal card 50.

Felling the last of the creatures, silence returns to the clinic, except for the perpetual cries emanating from the tower. "There was no saving those here," you tell yourself as you turn to leave. "Avenging them is the best we can now do."
Distribute the Blood Stone Shard reward among the Hunters.
Insight Mission – The Chosen ("H-Hunter. You've returned")
"H-Hunter. You've returned..." The girl from before moves toward you, as if in a trance. "P-Please. I know where we must go. F-Follow me. We... Haven't much time." She grips her stomach, an expression of pain taking hold of her. "Quickly now..."
Place 1 Survivor token on the Iosefka's Clinic space. Any Hunter may pick up the token when they move out of that space. The token is returned to the Iosefka's Clinic space if the Hunter teleports or goes to the Dream.
End a move on the Central Lamp tile with the token: Reveal card 51.
Insight Mission – The Chosen ("Mist suddenly swells around you")
Mist suddenly swells around you, consuming you both. As it clears, you find yourself back in the tower. Unphased, the girl staggers onward. "I-I needed it to stop... The constant writhing... My prayers were heeded... We must continue..."
Move your Hunter to the Mergo's Loft [blue orb] space. Then, place the Survivor token in that space. Respawn all Enemies on that tile. Each time a Hunter moves out of the space with the Survivor token, they may move the token into their space. Enemies suffer +1 [red drop] while in the space with the token.
