To surpass, do I have to beat every card played, or only the lead card?
The rulebook states this directly.

Only the lead card. Surpassing compares your card to the lead card, which never changes, so several players can surpass in the same round and each uses all the pips on their card. Cards played in between don't matter, and the lead suit stays the same even after a pivot.
