As soon as its trigger happens, even in the middle of an enemy's action. Side-Step, for example, triggers the moment the enemy becomes adjacent. To ignore an attack you wait until the dice are rolled, since that is when the attack is acted on you. If a Response changes an enemy's position, it recalculates its movement from there.
