How do Kalthak's Pincushion, Bone Prison walls and Dark Reflections work?

Pincushion moves him as far from all heroes as possible while staying in range of Bone Prison, with players choosing the exact square and direction. Neither Kalthak nor a Dark Reflection moves through bone walls. A Dark Reflection appears adjacent to the knocked-out hero, or in the closest free square. The knocked-out hero does not refresh a hand, so the reflection runs out of actions but still must be dealt with before that hero is healed. A bone wall does not spawn if a hero on that square cannot be pushed back one.
