Does a propagation happen every turn, and how many per turn?

No. Propagations happen only on turns when a virus is activated, a Propagation symbol is triggered, or you cross off your 2nd or 3rd System Error. Each virus activation (including one caused by a mission) triggers its own propagation, in which every active virus spreads once, and active viruses stay active for the rest of the game.
