Do I need a King's Order to refuse an Outsider, and when do I get the 3 coins?

Refusing and taking the top-right bonus requires trashing a King's Order (or another card from hand) except for leaders who welcome/refuse for free. The 3 coins come only from destroying a King's Order, so a free refusal destroying nothing gives none. Welcoming does not resolve the top-right effect.
