When I gain a card that replaces one in my grid, what happens to the replaced card?

The new card takes the replaced card's grid position and the replaced card goes to your reserve if there is room. The Equip action is different: its storybook entry tells you what to do and does not involve the reserve. A "when placed" benefit only applies when you gain a card directly into your grid.
