It is rolled when acquired and held as a counter. Each negative effect die that would land on you instead lowers it by 1 and is negated, and it is exhausted at 0. It can block a new Poison even while you are already poisoned (you just stay at the same Poison level). Players read the text as mandatory, so you can't save the uses.
