When do players refill their hands to six tiles?

Only at the end of the active player's turn, after both actions; every player below six then draws back up to six. The one exception is the replace-tiles action, which draws immediately. Nobody refills between actions or between conflicts, so a player who spends tiles in a conflict fights the next one with a smaller hand.
