The Loop
After a Vortex forms, what happens to the Rifts, and can more be added there?
The rulebook states this directly, and forum posts agree.

All Rifts on that Era, including the ones that caused the Vortex, go back to the pool. The Era keeps working and new Rifts can be placed there again. Only a second Vortex tile on the same Era (or a 4th overall) loses the game.
