Is the first-game setup supposed to differ from the normal setup?

Yes, on purpose. The first-game setup uses no Starting tiles and gives fixed resources, temple advances and worker positions to each seat, so new players start with balanced positions; for example the blue temple step gives fixed resources there instead of a free choice. Standard cocoa and player-order setup still applies.
