When do you reshuffle a discard pile into an empty deck?

Only when a card actually has to be drawn or added and the deck is empty. If your deck is empty and a card is put on top of it, that card is your whole deck until a draw forces a reshuffle. The galaxy deck works the same way, triggered by filling the galaxy row.
