Does the first player change every turn, or only when someone completes a card or coin row?
The rulebook states the first player changes after Step 3 of each turn.

Every turn. After Step 3 of each turn the first player role passes to the left, whether or not anyone completed anything. Some players misread the wording as applying only on completions.
