Are the unused start worlds shuffled into the deck at setup?

Yes — in the standard setup, the start worlds nobody was dealt are shuffled together with the game cards before the six-card hands are dealt. This is the base rulebook's procedure; the optional choose-your-start-world variant handles dealt-but-unchosen worlds differently (see the related entry).
