Does the game end immediately when a player places their 12th tableau card or the last VP chip is taken?

No. The game ends at the end of that round, after the remaining phases and everyone's discard to the hand limit. Players can therefore finish with more than 12 cards, for example by placing both a development and a world in the final round.
