What is the difference between an Exposed and a Contaminated city?

Exposed means a failed trial created an infection card for it; Contaminated is a separate, end-of-game status given to a city holding an Outbreak marker. A city can have both, and they are counted separately. Cubes alone don't contaminate, and if the game ends because you ran out of Outbreak tokens no extra marker or sticker is added.
