Count line segments to the hospital ignoring railroad tokens to find the closest patient; each moves one step toward the hospital, never ending farther away. A patient on a rail line moves as far along it toward the hospital as possible, without stopping at intermediate cities, so those do not receive anything. Ties are the players' choice.
