If a chain-reaction outbreak hits a city whose own Infection card comes later, does it outbreak again?
The Outbreaks rule limits the exemption to the current card.

Yes. The 'already had an outbreak' exemption only covers the current Infection card, so when that city's card is drawn it can outbreak again.
