Is grouping in lakes and loading zones required, and what is the size limit?
Not required — it's a choice there, and ships in a lake move individually. Any group totals at most four slots, can include ships from the same and adjacent loading zones of one ocean (all three boxes), and costs no action to form. Groups don't persist in lakes: you may reorder and leave some behind.
