After the skull is placed you immediately draw a new human from the base camp deck. If the human deck itself runs out, the rules are silent; the designer would reshuffle dead humans rather than eliminate a player. Heroes and special people without a green back (Draca, Jhula, Ahab, Orru) are not game humans, so if only they remain you also draw a new human.
