What does the Wraith's Drain do if I have no items, and how does it handle leveling?

With no items, Drain does nothing and the fight goes on. If Drain pushes you over a level threshold you level up after the encounter, and in a 2-player game it affects only one hero. A card with an item and a heart counts as one item.
