What happens if I have to spend time when the deck has no cards left?
The rulebook's Descending section says time spent while the stairs are visible goes on the stairs card, three tokens per damage; the 2015 designer reply predates this.

While the stairs card is visible (the deck is empty), each time you spend goes on the stairs card as a damage token. Every third token removes the three and one hero takes one damage, which can happen several times. In 2-player games either hero may take it.
