Does an expert campaign have to be played on expert villain difficulty?

No. Campaign difficulty is separate from villain mode, since an expert campaign only adds cards or setup instructions, so it can be played on standard or heroic modes too. An earlier reading that tied them together is superseded by the newer rulebook Q&A.
