One — against the in-range monster with the highest horror rating, and only for investigators in range when the Horror Step begins. Investigators sharing that monster resolve the same check. Horror checks come at the end of the Mythos Phase when the app tells you, not the moment a monster enters your space, and monster movement afterward does not trigger more.
