If my deck is empty and a card tells me to reveal or draw, do I reshuffle first?
The rulebook says not to reshuffle until the deck has completely run out and you still need to draw or reveal.

Yes, but only at the moment you actually need to draw or reveal more cards than the deck holds. Then shuffle your discard pile into a new deck and finish the effect.
