When exactly do survival opportunities exist?

Survival opportunities open at four kinds of moments: every flow on a monster AI card, the gaps between actions on the survivors' turn, two points inside another survivor's attack (after the wound roll but before the reaction, and after a critical wound's effects), and whenever a monster is knocked down. Nothing opens after hit locations are drawn but before the first wound roll, nor before the monster draws its first AI card.
