When is a survivor insane, and does being insane stop them using survival?

A survivor is insane while they have 3 or more insanity, so they can move in and out of insanity as it changes (unless a disorder keeps them insane). Insanity doesn't itself stop survival, but some effects do, such as the Immortal disorder, which bars survival while insane. Abilities that don't spend survival, like Sweet Battle, still work.
