How does brain damage work with insanity, and when do I roll on the Brain Trauma table?

Insanity is the armor of the brain: each point of brain damage reduces insanity first (and losing insanity counts as suffering brain damage), then fills the single brain injury box, and further damage makes a Brain Trauma roll. Brain damage is not dodgeable, and each hit rolls on its own, but all the damage of a single hit is applied at once. Brain trauma results above 10 come from bonuses to the roll.
