When does a demand count as having failed to do anything (for example for Hammurabi's karma)?

A demand counts as unsuccessful only when the demander knows for sure in advance that nothing will happen. A success requires something to change in the game state (or in game knowledge, such as a revealed card). If a change is still possible, for instance through a choice or coin flip not yet made, the demand counts as having had an effect.
