Can you reuse an exhaust token on a card you already exhausted, and does a passive card effect need the card to be played again?

Yes, you can re-use the exhaust token on that same card. Effects not preceded by 'passive' only happen when the card is played, so a card played earlier with Shinto in play does not require abandoning Tendai when Tendai is played later.
