How do cards showing both the civilised and uncivilised icons count?

For setup they are treated as uncivilised: they are shuffled with the uncivilised cards and get no progress token. Once the game starts they are both suits, so they can be acquired by either an acquire-civilised or an acquire-uncivilised effect and they count for both suits in end-game scoring.
