Do I choose my initiative card before the monster actions are rolled, and can I use either card?
The rulebook text states this plainly.

Yes. You select your two cards and decide which one sets your initiative before the monster dice are rolled, and the initiative can come from either of the two cards. On your turn you then play the top action of one and the bottom action of the other in any order; which card set initiative no longer matters.
