Can a player earn a milestone twice, and when do abilities start?
The designer said no milestone is gained twice and endorsed a per-milestone timing list.

No. Each player can earn a milestone only once, and one-time placement bonuses apply only on first use. Timing differs per milestone: some abilities are active the turn earned, others only from the next restructuring, payday or Clean Up. Another player can still earn the same milestone the turn it is first earned.
