How do keys and excess damage work in Stage D and D+?

In Stage D only the top Key is gained at a time; damage past the Key is ignored and the GK disc resets to 20 for the next one. In D+, the publisher says only the third Key (the one stacked above the last) can cost excess-damage discards, and the final hit must be exact or the disc stops one short. The scenario ends at the fourth Key regardless of remaining Gatekeeper hit points.
