Must a number placed on a Skull follow normal placement rules?

Yes. The covering card obeys the Field row rules and any Eternal curses, and the Gatekeeper is bound by them too. The Skull adds an extra condition (usually a colour) on top, and the no-adjacent-same-colour rule is waived for a card placed on a Skull because it was already checked when the Skull was placed. A Skull may also follow another Skull under the usual rules.
