Do the Treasure piles from Dominion and Intrigue have to be combined at 5-6 players?
No. The designer ruled that combining the Treasure piles at 5-6 players is optional, even though the printed 5-6 player setup says to combine them. Players report that you need extra Estates for the fifth and sixth starting hands, and that without the base set you use 15 Provinces and 40 Curses, improvising any missing Coppers or Silvers.
