Do I have to carry out all three actions, or can I do fewer?
The rulebook describes one simultaneous selection and resolution per action, with each action phrased as optional.

You choose a tile each time, but each action is optional ("may"), so you can do less than the full action or nothing at all. Tiles are chosen and resolved one action at a time in the order Build, Hire, Admit, with new patients drawn between actions, not all three tiles at once.
