What does the bonus tile with a white patient and three people with an arrow do?
The designer answered this in several threads, and the rulebook says bonus-tile people come from the supply.

It gives you one white patient, taken from the general supply (not the patient bag), placed in your Psychiatry pre-admissions area. The tile only says where the patient starts; nothing else shifts, and the arrow is a leftover pre-admissions symbol that never made it onto the player boards.
