Can a Dragon Attack happen during setup or before the first player enters the dungeon?
Follows the rulebook's setup step L; later attacks are covered by an official FAQ entry cited in the forum.

Not during setup: any card with a Dragon Attack symbol dealt to the opening row is replaced and shuffled back. But once play starts, an end-of-turn refill that shows a Dragon Attack still attacks everyone, including players who have not yet entered the dungeon (cubes of their colour in the bag can hit them).
