If I am insane, do I still have to play The Entertainer or Shoggoth when holding a 5 or higher?
A publisher account confirmed the 7s apply while insane, and the rulebook wording agrees.

Yes. The 7s' forced-play effect works from your hand, so being Insane does not switch it off. If you hold both 7s you may pick which to play, and Shoggoth is forced if you hold it with a 5 or higher such as The Musician.
